If your insurance is Amerigroup, Molina, Superior, Parkland, Aetna Better Health, or Ambetter, we do not take your insurance YET. We will be soon, so please check back.
You are about to complete our online scheduling process.
We suggest you use your smartphone because you will be asked to capture images of your current driver’s license or other government-issued photo identification and current insurance card. Please have these items available as you complete the online scheduling process.
Also please complete all steps throughout the health questionnaire. Failure to complete the online registration process will result in delays with your appointment.
A member of our office staff will contact you prior to your appointment to complete your registration process and verify your insurance.
You will receive a text message with a link to join the telemedicine visit 15 minutes prior to your appointment. As soon as you receive the link, click on the link to access the waiting room. Your provider will be with you shortly. Please make sure that you have a strong internet connection and you are in a quiet private area before your appointment begins.
Telemedicine is a mode of delivering health care services to our patients remotely so the patient does not have to physically come to our office. This mode of service allows providers to consult and treat patients through use of a computer or smartphone, while the patient is able to remain in their own home or office.
All urgent care type visits will be offered via telemedicine. If at any time during the visit your provider determines there is a need to see you in person, the visit will be converted to an office visit. (Note: if your telemedicine visit is converted to an office visit, you will only be charged for one visit, not both.)
